The form got refreshed in 2025.
If you are working off an old PDF or an old template, the layout and some of the wording will not match, and a payroll that looks off-template invites a closer read.
Nothing about the core duty changed. You still owe the prevailing wage and the fringe for the right classification, you still sign the statement of compliance, and you still file weekly. What changed is the paper. Make sure you are on the current form and that your template matches it.
